The PHA for VBC will work closely with Somatus patients and physician practices, including spending time working from within physician offices and will be the first and primary representative of Somatus, responsible for establishing trust and building relationships with the partnered physicians and practices. How You'll Make an Impact: The Patient Health Advocate (PHA) role supports Somatus high needs Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD) populations that face multiple challenges, from accessing resources to adhering to a physician's treatment plan.
KNOWLEDGE, SKILLS, AND ABILITIES: General practical and theoretical knowledge of the variety of laboratory procedures applicable to the area(s) of work; General practical and considerable theoretical knowledge of laboratory equipment, instrumentation, and terminology; General knowledge of the basic pathophysiological processes being monitored and how the medical laboratory science is applied in the medical field; Skill in the use of laboratory equipment and in the performance of procedures; Ability to make accurate observations and written reports of test results; Ability to understand and follow oral and written instructions; Ability to instruct students and other health professionals in laboratory procedures and instrumentation; Ability to work as part of a team; Strong computer skills including Microsoft Excel and Laboratory Information Systems. MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS: Associates degree in medical technology from an appropriately accredited institution or completion of a certified laboratory assistant course in medical technology or a comparable course, and one year of medical laboratory experience; or high school or General Educational Development diploma and three years of medical laboratory experience; or an equivalent combination of education and experience.
Performs duties as assigned by Pharmacy Manager, Staff Pharmacist and Store Manager including utilizing pharmacy systems to enter patient and drug information, ensuring information is entered correctly, filling prescriptions by retrieving, counting and pouring pharmaceutical drugs, verifying medicine is correct, and checking for possible interactions. May assist pharmacist in administering clinical services including the collection and proper labeling of blood/urine samples from patients and other clinical services as required; assists pharmacy staff in coordination of clinical services, Walgreens healthcare clinics and external providers.
Perform a variety of the technical procedures involved in the procurement, processing, and evaluation of patient specimens for use in the diagnosis and treatment of disease; teach and perform routine clerical and technical procedures including specimen collection, and responsible for the delivery of supplies and procurement of specimens from physicians' offices, clinical specimen preparation and processing for analysis and dispatch, patient registration and order entry; and serve as hospital laboratory representative and liaison with outside clients.
