Essential Functions: Provide expanded transitional case management services for transitions from incarceration, rehabilitation facilities, and/or homelessness to Denver Health's clinical resources (20%) Establish/strengthen the relationship between the individual and the support services needed after reentry, housing security, and/or clean, sober living (20%) Direct the selection of goals, interventions, and services based on patients' needs (20%) Provide wrap-around services to ensure a successful transition back into the community (20%) Create processes for individuals becoming Denver Health patients so they are connected with Enrollment Services, the Appointment Center, and other departments, as needed.
