The Administrative Coordinator utilizes collaboration and project management skills to assist with assessment, planning, implementation and evaluation to assist Case Management, Social Work and Utilization Review staff with coordination of care in the Primary Care setting.

Essential Duties and Responsibilities

  • Collaborates with multiple departments and provides high-level project support and analytical services focusing on quality initiatives and patient care management
  • Requires strong multi-tasking capabilities
  • Requires adherence to strict confidentiality guidelines
  • Assists with obtaining Durable Medical Equipment (DME), including but not limited to rolling walkers, commodes, wheelchairs, quad canes and hospital beds.
  • Make basic transportation arrangements (MAS, Rehab, back transfer, cabs) once CM/SW has documented need.
  • Maintains patient confidentiality of all communications and documents as required by hospital policy.
  • Keeps Case Managers and Social Workers apprised of status of discharge plans, documents the status of activities and communications, and maintains adequate reporting information in EPIC.
  • Maintains daily documentation in EPIC of all activities done utilizing interventions.
  • Enters data into EPIC in a timely fashion to facilitate discharge plans.
  • Maintains skills needed for performance of duties.
  • Utilizes available educational opportunities for improving skills.
  • Attends required in-house in-services.
  • Projects positive image of the department and hospital.

Qualifications

  • Associate's degree - required
  • Prior experience in customer service.
  • preferred
  • Working knowledge of computer-based applications and strong PC/ keyboard skills.
  • Knowledge of medical terminology.
  • Strong attention to detail and demonstrated ability to use sound judgment in decision making.
  • Excellent organizational skills with demonstrated time management skills.
  • Ability to handle high workload volume with many variables, demonstrating flexibility and adaptability.
  • Excellent customer service skills (written and verbal), demonstrating the ability to work effectively and efficiently with a diverse population.
  • Ability to work well independently as well as within a team unit.
