p>The RN Case Manager/Utilization Review is responsible for performing prospective, concurrent, and post‑discharge utilization reviews to ensure appropriate patient status, medical necessity, and compliance with hospital policy, payer requirements, and applicable local, state and federal regulations, including Centers for Medicare & Medicaid Services (CMS) guidelines. - Monitor, track, and analyze avoidable days and extended lengths of stay; identify contributing factors related to utilization, payer processes, discharge barriers, and system delays, and collaborate with Case Management, physicians, and interdisciplinary teams to support timely resolution.
This role will support the continuity of care across the continuum by advocating for the needs of the patient and family and working with identified care team members to promote access to care, facilitate communication and provide effective resource coordination during care transitions to ensure continuity, quality and closure of gaps in care. Uses clinical judgment based on nursing skills acquired through formal and informal experiential knowledge and evidence based guidelines to globally assess the patient's situation and through critical thinking and clinical decision making, develop an appropriate plan of care for the patient, with the aim of promoting best outcomes.
Counseling/Consultation: Consult and advise student support coordinators, Whole Child Site Coordinators, campus counselors, and principals as requested regarding student and/or family issues related to substance abuse, suicide threats, domestic violence, CPS, bullying, etc. Collaborate with the Guidance and Counseling team to update the referral and community resources list for the District, including specific point-of-contact information for various community agencies, including resources for free or reduced counseling for students and families.
