Collaborate with inpatient pharmacists, inpatient nursing, pharmacy financial counselors, and the meds to beds outpatient pharmacy staff to assist in ensuring patient discharge needs are met in a timely manner. Utilize available electronic resources, patient interview, and phone follow up to ensure an accurate medication history is available for patients at admission or upon presentation to the emergency department.

Proactively consults with the interdisciplinary team which includes but is not limited to hospital patient care staff, physicians, patient support and family and community resources to ensure a smooth transition for the patient through appropriate levels of care in order to facilitate quality outcomes. Facilitates timely referrals to physicians (consults), ancillary services/programs (e.g., Pastoral Care, Nutritional Services, Diabetes Educators) for individualized assessments/interventions based on initial and ongoing assessments.
Certified Clinical Hemodialysis Technician (CCHT) required. Dialysis Technician (CCHT) – Dialysis Center (Days).
li>Performs all tasks necessary for the safe, smooth, and efficient operation of the unit – this may include, but is not limited to, water checks, opening and closing of the unit, mixing bicarbonate and acid, bath distribution, maintenance of the water treatment system and assisting with stocking of supplies. In addition to the healthcare team, you are also required to maintain strong relationships, regular communication, and visibility with clients (site Managed Healthcare, DOC/BOP, etc.).
