Coverage Details:Coverage Area: North Kansas City including Liberty, Platte City, Excelsior Springs, Blue Springs, and IndependenceAverage Daily Mileage: Approximately 50 to 100 miles per day, typically 50–70 milesMileage Reimbursement: $0.45 per mile, excluding mileage to and from homeKey Responsibilities:Provide hospice and home health social work services through in-home patient visitsConduct psychosocial assessments and develop care plans tailored to patient and family needsSupport patients and families with counseling, education, and resource coordinationCollaborate with interdisciplinary team members to ensure comprehensive care deliveryAssist patients and families with advance care planning and end-of-life decision-makingDocument assessments, interventions, and care plans using electronic charting systemsMonitor patient needs and communicate changes to the care teamPromote holistic, non-pharmacologic approaches when appropriate, addressing nutrition, lifestyle, and psychosocial factorsMaintain compliance with all federal, state, and organizational regulationsRequired Qualifications:Master of Social Work (MSW) degree from an accredited programActive LMSW license in Missouri and/or Kansas, as applicableMinimum of 1 year of experience working in a healthcare setting, hospice experience preferredCurrent CPR certificationValid driver's license and active auto insuranceU.S. The LMSW plays a critical role in supporting patients and families through psychosocial assessments, care planning, education, and emotional support while collaborating closely with interdisciplinary care teams.
