I worked as a social worker for eight years and I saw first hand the challenges of finding reliable, trustworthy, affordable and life enhancing home care so I became a nurse to better understand and help more people. I like to share with you all the great things that my caregivers and I do to help and support caregiving families but first a little bit about me and how this service came about.

Qualifications: High School Diploma or equivalent Five years' experience working in the construction or home improvement industry Basic carpentry, painting, plumbing, and electrical skills, some basic computer skills, trade or vocational training, RRP qualified, knowledge of building codes and OSHA training a plus. Coordinates with the Care Coordination team to ensure that the EMR reflects the education, coordination of home care services, and placement in an extended care facility, durable medical equipment, and referral to complex care management team, ACO navigators and authorizations from providers. Responsibilities: A day in the life of a Trauma Social Worker at Hackensack Meridian Health includes: Occasional physical demands include: Climbing (e.g., stairs or ladders) , Carry objects , Push/Pull , Twisting , Bending , Reaching forward , Reaching overhead , Squat/kneel/crawl , Wrist position deviation , Pinching/fine motor activities , Keyboard use/repetitive motion , Taste or Smell , Talk or Hear.
