I worked as a social worker for eight years and I saw first hand the challenges of finding reliable, trustworthy, affordable and life enhancing home care so I became a nurse to better understand and help more people. I like to share with you all the great things that my caregivers and I do to help and support caregiving families but first a little bit about me and how this service came about.

Our facilities and sites of care include: Morristown Medical Center, Morristown, NJ Overlook Medical Center, Summit NJ Newton Medical Center, Newton, NJ Chilton Medical Center, Pompton Plains, NJ Hackettstown Medical Center, Hackettstown, NJ Goryeb Childrens Hospital, Morriston NJ Atlantic Rehabilitation Atlantic Home Care and Hospice. div class="job-description-container"> p>Responsibilities: • Perform Maintenance tasks • Submit required documentation to office personnel • Ensure timely completion of tasks • Follow up with staff and consumers when necessary • Maintain safe working conditions • Coordinate activities with supervisory staff • Supply and maintain your own basic hand tools. Qualifications: High School Diploma or equivalent Five years' experience working in the construction or home improvement industry Basic carpentry, painting, plumbing, and electrical skills, some basic computer skills, trade or vocational training, RRP qualified, knowledge of building codes and OSHA training a plus. Coordinates with the Care Coordination team to ensure that the EMR reflects the education, coordination of home care services, and placement in an extended care facility, durable medical equipment, and referral to complex care management team, ACO navigators and authorizations from providers. Responsibilities: A day in the life of a Trauma Social Worker at Hackensack Meridian Health includes: ul> Occasional physical demands include: Climbing (e.g., stairs or ladders) , Carry objects , Push/Pull , Twisting , Bending , Reaching forward , Reaching overhead , Squat/kneel/crawl , Wrist position deviation , Pinching/fine motor activities , Keyboard use/repetitive motion , Taste or Smell , Talk or Hear. Coordinates transitional care to ensure continuation of care by assisting participants with reinstatement of conventional Medicare and Medicaid benefits, transitions care to other providers, makes referrals, and ensures participant medical records are available to new providers in the event of participant termination of the PACE Program. Coordinates with the Care Coordination team to ensure that the EMR reflects the education, coordination of home care services, and placement in an extended care facility, durable medical equipment, and referral to complex care management team, ACO navigators and authorizations from providers. To excel as an Individual Support Worker at Owen Health Care Inc., candidates must possess a college degree with one year of experience or a High School Diploma/GED and a minimum of three years of supervised experience in implementing positive behavior support plans and teaching ADL/Instrumental skills to individuals with intellectual and developmental disabilities. We provide compassionate, quality, and affordable health care services to adults with mental health, substance abuse, and co-occurring disorders, as well as to all individuals with disabilities-including children and the elderly. To excel as a Bi-Lingual Individual Support Worker at Owen Health Care Inc., candidates must possess a college degree with one year of experience or a High School Diploma/GED and a minimum of three years of supervised experience in implementing positive behavior support plans and teaching ADL/Instrumental skills to individuals with intellectual and developmental disabilities. As a Part Time Bi-Lingual Individual Support Worker at Owen Health Care, you will play a pivotal role in assisting behavioral therapists to create effective behavioral therapy programs tailored to children and adolescents. p>A day in the life of an Licensed Clinical Social Worker at Hackensack Meridian Health includes: The CHW will be: working to improve patient health by providing social support, advocacy, and resource navigation; conducting health and wellbeing check-ins at clinics, community sites, patient homes and remotely by virtual visits and phone calls; connecting patients with follow-up clinic appointments, social resources, reinforce diabetes and hypertension health education and collaborate with patients to identify barriers to chronic disease management and work toward solutions; building strong relationships with patients and their families.
