Partner with the interventional cardiologists, cardiothoracic surgical team, and clinical staff of multiple departments, both inpatient and outpatient within Baystate Health and external local and regional healthcare facilities to coordinate the care of cardiovascular patients who are being evaluated and selected for cardiovascular procedures through their pre- and post-procedure care. Reporting to the service line director, you will be responsible for coordinating and leading the planning and implementation of clinical initiatives to improve the quality, efficiency, and cost-effectiveness of care and will be instrumental in creating workflow process for electrophysiology procedures, atrial fibrillation referrals and and regional outreach to providers for EP referrals.
As a Case Coordinator at Hartford Hospital, assigned to a select inpatient population, you will assess patients for appropriate level of care; coordinate their care and services across the care continuum; identify risk factors; facilitate involvement of support services; perform utilization management functions; develop and implement transition planning activities; utilize and document aspects of care in our case management specific computer applications; and collaborate with other health care team members on process and quality improvement initiatives. Three years acute-care, and diversified clinical nursing experience, including medical/surgical nursing is required; case management, home care, and/or managed care experience strongly considered.
Provide leadership, planning, implementation, and management of performance and quality improvement programs that help assess and improve operational performance, contract stability, patient satisfaction, financial viability, and knowledge dissemination for the practice line. • As a liaison between the provider group and other members of the health care team with a focus on Case Management and nursing allowing the provider to function at the highest level of the scope of their role.

p>The Patient Service Coordinator is responsible for administrative support in a busy medical office setting providing duties from scheduling appointments, verifying insurance precertification, as well as triaging phone calls from patients, providers, and insurance companies, while maintaining patient and business confidentiality to ensure that the needs of the internal and external customers are met.
