div class="job-description-container">We are seeking dedicated and compassionate individuals to join our team as Case Managers (CM) and Community Based Rehabilitative Specialists in Idaho. Born from personal experience and with a mission to instill hope, One Love now provides general therapy, case management, community based rehabilitation, peer support, and payee services.
p>• Ability to stand and/or walk 60% of the time • Ability to assist in lifting patients over 100lbs occasionally and frequently lifting equipment and other items up to 25lbs • Ability to frequently stoop, crouch and/or reach below, at, or above shoulder level for short periods of time • Manual dexterity, ability to concentrate, and good eye-hand coordination required • Vision requiring near and far acuity, depth perception, and an ability to adjust focus and maintain a good field of vision is required for patient assessment, observation, treatment and documentation • Mental, Physical, and emotional abilities to respond to emergency situations • Written and spoken communication skills require the ability to speak, hear and see • Critical thinking and good judgment needed to be able to follow Federal, State and hospital policies and procedures. Physicians in this role practice full-spectrum family medicine, including:
• Comprehensive outpatient primary care for adults and children • Preventive care and chronic disease management • Same-day acute care visits when hospital coverage allows • Inpatient rounding and hospital admissions • Collaboration with emergency, surgical, and specialty services • Hospital follow-up care and care coordination • Opportunities may exist to incorporate additional services based on physician training and interest, such as women's health, procedural care, etc.
p>Commission on Collegiate Nursing Education CCNEAccreditation Commission for Education in Nursing ACEN and the American Nurses Credentialing Center ANCC American Academy of Nurse Practitioners Certification Board AANPCB National Certification Corporation NCC offer certifications which are themselves accredited by bodies like the Accreditation Board for Specialty Nursing Certification ABSNC and National Commission for Certifying Agencies NCCA. HeShe will be required to exercise sound professional clinical judgment while providing primary health care services to ambulatory patients newborn to end of life patients geriatric patients womens health patients within the specified population served by Terry Reilly clinics.
