Under general supervision and in accordance with established department policies, procedures, care protocols, and provider orders, this position will work within the Patient Centered Medical Home model to systematically analyze patient health information to identify and coordinate the care needs of the individual patient focusing on the continuity of care. Ability to handle stress related to demanding or difficult patients/team members, competing priorities, and changing processes.

Oversees and completes the "make-ready" process to prepare vacant apartment homes for leasing and new move-ins by completing the pre-move-out inspection, creating a "punch" list of maintenance work needed, scheduling vendors and contractors as needed, obtaining needed supplies and materials, completing all maintenance tasks, and inspecting completed work. Proficiency in customer service and interpersonal communication skills in order to effectively interact with residents, clients, team members, and other business contacts, respond courteously to questions and requests, and stay calm when addressing and resolving customer problems.

Perform general carpentry duties such as wallboard repair, countertop/cabinet replacement, finish carpentry, window/glass repair, door/lock repair, and ceramic tile repair. Perform various maintenance functions including repairs to the following: electrical, plumbing, appliances, flooring, carpentry, HVAC, grounds maintenance as dictated by state guidelines.
Owns the pharmacy manager talent strategy and pipeline, executing the recruitment strategy, identifying and coaching high potential pharmacy managers, and managing Performance Improvement Plans for Pharmacy Managers. Accountable for pharmacy talent pipeline, including leading recruitment, development, retention and performance of Pharmacy Managers, Staff Pharmacists, technicians and the Pharmacy scheduler.
