The RD-Home Infusion Clinical Liaison serves as the nutrition liaison between referral sources, the home infusion pharmacy, and the interdisciplinary care team to support timely initiation and ongoing management of enteral nutrition therapy. Provides education and counselling to patients and caregivers on enteral feeding therapy, including formula administration, pump use, tube care, troubleshooting, and nutrition goals.
Shares clinical and scholarly activity that promote the evidence-based use of medications and advance the profession of pharmacy to a variety of audiences (represents Prisma Health within professional organizations through service, presentations or publications; provides education within didactic and experiential training programs and to postgraduate professionals, nursing and medical staff).
