Job Type: Permanent / Direct Hire
Schedule: Days | Monday–Friday, 8:00 AM–5:00 PM
Salary: $120,000–$170,000 per year
Start Date: September 28, 2026
On-Call: Yes — 24/7 responsibility with a focus on developing leaders to minimize call escalation

About the Opportunity

We are seeking an experienced and collaborative Director of Care Management to lead hospital and ambulatory care management programs in Des Moines, Iowa.

This is a strategic leadership opportunity for a healthcare professional who can drive integrated care management initiatives, develop high-performing teams, improve patient outcomes, and strengthen coordination across the continuum of care.

The Director will serve as a key subject matter expert and strategic partner to clinical, operational, and external stakeholders while helping advance organizational quality, efficiency, and patient-centered care goals.

Key Responsibilities
  • Provide strategic leadership and operational oversight for Integrated Care Management (ICM) programs across hospital and ambulatory settings.
  • Lead multidisciplinary teams of RN Care Managers, RN Care Coordinators, and Licensed Social Workers.
  • Partner with physicians, clinical leaders, community organizations, and cross-functional teams to promote seamless care coordination.
  • Drive initiatives to reduce readmissions, improve patient flow, optimize length of stay, and enhance care management outcomes.
  • Monitor and improve clinical, operational, and financial performance metrics.
  • Ensure compliance with CMS, DNV, OSHA, OIG, and applicable federal and state regulations.
  • Coach, mentor, and develop leaders and team members while promoting accountability, collaboration, and service excellence.
  • Allocate resources effectively and identify solutions to operational barriers.
  • Serve as the market subject matter expert for care management strategies and initiatives.
  • Participate in an on-call structure and provide 24/7 leadership support as needed.
