Specifically, the Clinical Care Specialist: Makes appropriate referrals to address mental health and substance use needs Provides behavioral health counseling, group treatment, and resource-based case management Collaborates with other treatment providers to ensure a team-based approach to treatment, when applicable Identifies coping mechanisms and natural supports that may be available Teaches and enhances client coping, symptom management, problem-solving, independent living, and socialization skills Develops, implements, and reviews treatment plans with clients Maintains competency and proficiency with our electronic health record system. Bachelor's degree in social work, psychology, criminal justice, or a closely related field Experience in case management/community systems navigation Experience working with persons diagnosed with severe & persistent mental illness and substance use disorders Exceptional flexibility, time management, and documentation skills Colorado driver's license, a good driving record, and access to transportation during work hours.
