Provide interpretation and problem solving of physician’s orders, checking for formulary status, appropriate drug selection, dose and dosage form, drug-drug interactions, drug-nutrient interactions, drug duplication, potential for adverse events, appropriate route of administration, therapeutic substitution, IV to PO conversion, or other formulary or departmental initiatives. Provide pharmaceutical care for patients, monitor drug therapy and coordinate drug distribution, ensure medication safety, participate in education of students and/or residents, and develop and maintain professional relationships with other healthcare providers.
