Identify and develop relationships with healthcare professionals involved in the care of heart failure patients (i.e., Physicians, APPs, Nurses, Social Work/Case Management staff, THC and other Cardiology Office staff, Home Health staff, SAR staff); provide educational opportunities to promote a better understanding of the patient navigator's role and to encourage referrals. Identify and develop relationships with community agencies (i.e. AAHFN,HFSA, AHA, Wellness Community, Legacy Link, United Way, Mom's Meals, Meals on Wheels, NGHS Home Health) involved in heart failure/elder care that provide personal health services, financial assistance, support and education.
Identify and develop relationships with personnel in departments involved in the care of the cancer patients (i.e. Physicians, Surgeons, Nurses, Radiology staff, Case Management staff, radiation oncology staff, hematology / oncology office staff); provide educational opportunities to promote a better understanding of the patient navigator's role and to encourage referrals. In conjunction with Physicians and other Healthcare professionals, the Oncology Nurse Navigator is responsible for, but not limited to, facilitating interactions between patients and their Physicians, providing logistical support, securing referrals, and assisting with financial and insurance issues.
p>Work Shift: JOB DESCRIPTION: Functions as a clinical navigator with a specific set of skills beyond licensure requirements to provide better patient care, quality, safety and informational accuracy for breast imaging care. Works with department and system leadership to create a dashboard with drivers for quality metric improvements within breast imaging that align with the defined radiology strategic plan.
Working collaboratively with physicians and advanced practice providers (APP), the Dementia Navigator helps coordinate medical, behavioral, and social services while providing education, counseling, and resource navigation for patients and caregivers and is critically responsible for ensuring impactful, high quality, meaningful outcomes and associated caregiver satisfaction and is the focal liaison for the Cognitive Care Center and the greater, global community. The Dementia Navigator serves as a central point of contact for patients and caregivers, ensuring continuity of care and connecting families with appropriate community resources and support services.
The Cognitive Nurse Navigator, a Registered Nurse, is responsible for the navigation of the patient through the continuum of healthcare during the patients time within the NGPG practice, and during the time in which they are under the care of an NGPG Neurologist for a cognitive disorder. Significant implementation includes planning, implementation, coordination, monitoring, and evaluation of individualized care options, services, resourcing, education and follow up needs.
