Responsible for serving on the ambulatory care team for an assigned area including direct patient care as an unlicensed professional and under the direct supervision of a Registered Nurse (RN), Physician or APP. Your role, within your scope of practice, for the comprehensive geriatric evaluation of patients will include, but not limited to, obtaining vitals signs and administering geriatric assessment tools followed by sharing these results with the provider. Comprehensive geriatric assessments may be conducted in-office, via telemedicine, or through on-site visits at senior facilities, patient homes, dialysis centers, and other appropriate locations.
