The APP practices independently and collaboratively with transplant physicians/ surgeons and interdisciplinary team members to assess, diagnose, treat, and manage complex transplant patients while supporting clinical operations, patient education, and interdisciplinary collaboration. The APP communicates care plans effectively with patients and families, provides specialty consultation, supports clinical pathways and protocols, and ensures patient advocacy, education, confidentiality, and high-quality, patient-centered care.
