Provides direct patient care when needed, evaluates outcomes, timely consults with other care providers as warranted and adjusts nursing care interventions appropriately to ensure optimal patient care, including placement of patients at the proper level of care. Desert Care Network serves the healthcare needs of the Coachella Valley and Morongo Basin regions in Southern California with three hospitals Desert Regional Medical Center in Palm Springs, JFK Memorial Hospital in Indio and Hi-Desert Medical Center in Joshua Tree.
Excellent communication (written, verbal, and phone) Operate as a self-starter who takes initiative Independently organize workload, follow-up schedules, and booking priorities Identify gaps in workflow or communication and proactively implement improvements Demonstrate personal accountability for results Maintain structured task management and booking systems Keep spreadsheets, CRM records, and internal reports organized and up to date Develop improved processes to streamline follow-up, rebooking, and scholarship tracking Think critically about efficiency and suggest system enhancements Strong working knowledge of CRM systems (HubSpot or similar) Proficiency in Excel (formulas, sorting/filtering, basic reporting) Comfortable learning and implementing new software tools Detail-oriented with good organizational skills Ability to multitask in a fast-paced environment. Leads the process of acquiring Forest Home Program contracts Initiates and participates in daily outbound phone calls and face-to-face appointments to secure new contracts and maintain existing contracts Manages the development of new partnerships via team prospecting of past customers, cross-selling, targeted lists, referrals, networking opportunities, conferences, donors & board members Provides camp tours for church partners as needed.
Entrepreneurial, proactive, and growth-oriented mindset Proven success managing donor relationships and achieving revenue goals Strong relationship building and interpersonal skills Confidence in making direct financial asks Excellent written and verbal communication skills including storytelling High emotional intelligence and active listening ability Strategic thinker with strong analytical and problem-solving skills Experience with CRM systems (Bloomerang and DonorSearch preffered) Ability to manage a large caseload with strong time management and prioritization Self-motivated, organized, professional, goal oriented Passionate about educating and inviting donors to join God's Kingdome work at Forest Home Solid understanding of fundraising analytics and donor behavior Strong project management and deadline adherence. Coordinate targeted year end and campaign asks aligned with broader development efforts Respond to new donor inquiries within 2 hours Maintain accurate and timely CRM records (Bloomerang, DonorSearch) Track KPIs: retention, upgrades, lapses, and pipeline movement Develop monthly performance dashboards and reports Monitor retention rates (target: 75-80%+) Develop and manage a comprehensive annual donor communication plan Align messaging with ministry impact and donor interests Coordinate with the development teams, marketing on appeals and storytelling.
