Supports and maintains sponsorship over multiple mission partner SCI indoctrinated individuals ensuring training and reporting, are executed properly at each partner facility.- Build and maintain relationships with community stakeholders,
- Establish and maintain resources and information relating to medical services/programs (e.g., Medicaid) and job opportunities,
- Actively participate in the Multidisciplinary Treatment Team (MDT),
- Complete a risk and needs assessment on youth as they enter the Program, and complete re-assessments within established contractual guidelines,
- Evaluate, develop and document an individual service/care plan (ISP) for the youth based on the assessments,
- Ensure youth receive applicable assessments within the required timeframe of enrollment and that this data is used in the development of the ISP,
