Supports and maintains sponsorship over multiple mission partner SCI indoctrinated individuals ensuring training and reporting, are executed properly at each partner facility. Set up and confirm orders submitted for distribution through PR Newswire including vetting content for acceptability, reviewing and confirming order details with clients to accurately pass information through a processing queue, counseling clients on additional product solutions, and providing effective communication both verbal and written to support clients through the distribution process. - Build and maintain relationships with community stakeholders,
- Establish and maintain resources and information relating to medical services/programs (e.g., Medicaid) and job opportunities,
- Actively participate in the Multidisciplinary Treatment Team (MDT),
- Complete a risk and needs assessment on youth as they enter the Program, and complete re-assessments within established contractual guidelines,
- Evaluate, develop and document an individual service/care plan (ISP) for the youth based on the assessments,
- Ensure youth receive applicable assessments within the required timeframe of enrollment and that this data is used in the development of the ISP,
