The social worker will assess for behavioral health needs and connect with resources, provide brief social work interventions, identify and troubleshoot any barriers to diabetes care, be involved in diabetes specific supports including support groups, complete psychosocial assessments, and assist with needs including housing, transportation, insurance, or other financial concerns. Frequent interactions with providers, colleagues, customers, patients/clients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately.
