p>Telecare Sonoma ACT (Assertive Community Treatment) is a community outreach program that serves 60 adults, ages 18+, with serious, persistent mental illness, and/or dual diagnosis.

Drives program development and management by: planning and coordinating adjustments to current therapy programs and future program implementation (e.g., in-/out-patient psychotherapy, chemical dependency) or new programs to enhance quality and effectiveness of services; building new systems and processes for outcome management as well as overseeing the implementation of improvements to clinical treatment programs, designs, or protocols; and analyzing the productivity of large, complex therapy programs for providers and ensuring the optimal utilization of programs. Manages designated units or teams by translating business plans into tactical action items; oversees the completion of work assignments and identifies opportunities for improvement; ensures all policies and procedures are followed; partners with key stakeholders and business leaders to ensure products and/or services meet requirements and expectations while aligning with departmental strategies.

Job Requirements: Education and Work Experience: Bachelor's Degree in nursing or equivalent combination of education/related experience: Required Master's Degree: Preferred Five years' home care nursing experience: Preferred One year's leadership experience: Preferred One year's hospital case management (including discharge planning) experience: Preferred Licenses/Certifications: Registered Nurse (RN) or Physical Therapist (PT) licensure in the state of practice: Required Public Health Nurse licensure in state of practice: Preferred Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy: Preferred Valid Driver's License (DL) and must be at least 21 years of age or older: Required Physical Therapist (PT) or Registered Nurse (RN): Required Essential Functions: Participates in discharge planning discussions with patient, patient care team and patient families. Serves as resource and liaison with hospitals, skilled nursing homes, board and care homes, outpatient discharge planning services and physician offices.

Responsible for managing day-to-day HSE field compliance; correlating and auditing operational activities against current permits; compliance with all environmental permitting requirements; OSHA safety compliance program; providing accurate and comprehensive health, safety, and environmental training requirements for the operational group. The employee is occasionally required to stand; use hands to finger, handle, or feel; reach with hands and arms; and may occasionally be required to climb or balance; and stoop, kneel, crouch, or crawl.
