Community Health Worker, The Freeman Center, Full Time, First Shift UC HealthCommunity Health Worker, The Freeman Center, Full Time, First ShiftCincinnati, OHThe primary goal of the Community Healthcare Worker will be to identify patients who are in need of primary care, "medical home" needs, community resources and care coordination to promote basic healthcare access and meet social, physical and developmental needs for a diverse group of patient populations. Members of UC Health include: UC Medical Center, West Chester Hospital, University of Cincinnati Physicians and UC Health Ambulatory Services (with more than 900 board-certified clinicians and surgeons), Lindner Center of HOPE and several specialized institutes including: UC Gardner Neuroscience Institute and the University of Cincinnati Cancer Center.
