The outpatient physical therapist provides an individualized evaluation, during which the client, family/caregiver, and physical therapist determine the patient''s goals, customize interventions to optimize the person''s ability to perform functional activities, and continuously assess progress towards goals to ensure goals are being met and/or to modify the intervention plan based on the patient''s needs and skills.
