Community of Hope is seeking a Family Nurse Practitioner to work closely with our physicians, nurses and other care team members to provide primary and preventive healthcare services to individuals and families across the lifespan, emphasizing health promotion, disease prevention, and management of chronic and acute conditions. Works with adults and children to develop case management plans that are updated at least monthly and signed by clients and case manager, which incorporate goals for both adults and children related to core goal areas: finding stable housing; increasing income; and family stabilization and support.

Minimum of two years of experience working in housing/homelessness or social services field;

Strong knowledge of community resources or ability to become knowledgeable required;

Strong interpersonal and organization skills.
