Essential Functions: Trauma-Informed & Therapeutic Care Provide case management services to youth experiencing the residual effects of complex trauma Utilize trauma-informed interventions and relationship-based approaches that support emotional regulation, growth, and healing Participate in intensive treatment planning for youth with high behavioral and emotional needs Facilitate therapeutic support to the youth Professionalism & Community Collaboration Maintain a high level of professionalism when working with courts, probation, external team, GALS, community providers, and family members Build strong partnerships with community stakeholders to ensure continuity of care and successful reintegration Represent youth needs professionally during court hearings, staffing meetings, Treatment plan meetings, and treatment reviews Attention to Detail & Documentation Complete reports, case notes, treatment updates, and required documentation accurately and on time Complete weekly check ins with caseload, document needs and support needs. This position plays a vital role in helping youth successfully transition into family, school, and community settings through individualized case management, advocacy, therapeutic support, and professional collaboration with internal teams and community stakeholders.
