Comprised of four hospitals (Moore, Hoke, Richmond, and Montgomery) with more than 600 beds, the system also offers leading-edge heart care in the Reid Heart Center, the area’s only dedicated heart and vascular center.
Identifies real/potential ethical issues, educates healthcare team members, and participates in resolution in collaboration with other healthcare team members • Proactively initiates end of life discussions with patients, families and/or physicians when appropriate. Responsibilities:
• Completes psychosocial screenings, assessments, and appropriate interventions in collaboration with patients/families, physicians, and other healthcare team members to meet care plan needs.
ul>Provides holistic and comprehensive case management services to all clients including intake assessment, biopsychosocial assessment, ASAM, utilization reviews, goal setting, and weekly progress monitoring. Maintains an active case management caseload, providing interventions as needed and within area of expertise and limits of credentials.
