PROGRAM: Veteran Services
REPORTS TO: Program Manager
INTRODUCTION:. Provide/refer for supportive services to participants, including personal financial planning services, transportation services, income support services, fiduciary and representative payee services, legal services, child care, and housing counseling.

The Care Manager will promote quality cost-effective outcomes managing care needs through the continuum of care utilizing effective verbal and written communication skills and a consumerism approach through education and health advocacy to members serviced.
Establishes a documented patient centric care management plan involving all appropriate parties (client, physician, providers, employers, etc), identifies anticipated case results/outcomes, criteria for case closure, and promotes communication within all parties involved.Body Movements: The employee must have a full range of body movements including the use of his/her hands, feet, and limbs to perform CPR compressions, abdominal thrusts, CPI (The Crisis Prevention Institute is an international training organization that specializes in the nonviolent crisis intervention training designed to teach best practices for managing difficult situations and disruptive behaviors.
