p/>At Saint Alphonsus, the Metabolic and Bariatric Surgery (MBS) Care Coordinator is responsible for coordinating bariatric patient care with providers, triaging bariatric patients (telephonically/in-person), conducting pre-visit chart reviews to assess and anticipate bariatric patient care needs, developing and providing patient education, coordinating care across the continuum, and supporting quality improvement activities within the practice at Saint Alphonsus Medical Group Metabolic and Bariatric Center.
Assists/develops written protocols and education of nurses detailing the rapid communication and basic response to critical vital signs that is specifically required to minimize delays in the diagnosis and treatment of serious adverse events for the metabolic and bariatric population.
