The Regional Manager is responsible for the implementation and delivery of the Population Health Care Coordination program which includes oversight and daily management of the Nurse Care Coordinators, Social Work Care Coordinators, Pharmacist Care Coordinators and Administration Assistants. They will oversee delivery of an integrated care management program based on individual needs of the patient to ensure we are meeting the Triple Aim +: appropriate care at the right time in the right setting with a goal of improving clinical outcomes and avoiding adverse events.
