Provision of leadership in developing and coordinating school-community partnerships that includes school-based and school-linked services • Development and implementation of intervention and prevention programs that will assist in the reduction of at-risk students • Participation in MTSS initiatives in the school environment to support student success • Referral to, and consultation with, appropriate community agencies; development and coordination of relevant community resources to address specific needs of the students and families • Linkage of services and intervention strategies to the classroom to enhance the child's overall educational development. • Provide evidenced based, crisis, individual and group interventions to students in need • Consultation services to faculty, co-team members, parents and other appropriate parties regarding the socio-cultural and emotional factors impacting on the student's learning experiences • Complete diagnostic assessment for students needing initial evaluation or reevaluation • Individual Education Conferences to help determine eligibility for special education programs and counseling services on all cases where social work services are recommended.

The scope of practice includes but is not limited to psychosocial assessments, Discharge planning, and providing referrals for needed services in collaboration with physicians, evaluates and implements treatment regime and discharge plans for designated medical patient populations. Assesses and diagnoses the psychosocial needs of the patients, caregivers, and families through initial history including medical, psychiatric, substance abuse, community resources, and ADLs.
p>QUALIFICATION SMSW degree from a school of social work accredited by the Council on Social Work Education Professional Educator License with Social Worker Endorsement and Approval and Registration as a Licensed Clinical Social Worker or Licensed Social Worker through the Department of Professional Regulation. p>Duties The Social Worker provides comprehensive school social work services to parents, students, and staff by addressing barriers that limit a student from receiving full benefit from their educational experience.
