This will be accomplished by developing a structure to effectively communicate, collaborate, and engage a team of peers and physicians in the ambulatory pharmacy strategy; ensure that measurement of the medication management system aligns with metrics focused on quality, safety, cost, and utilization; execute on a quality improvement plan that ensure the best outcomes through performance-improvement activities. 3. Knowledge of complex/chronic diseases states and therapies used within specialty pharmacy such as but not limited to: Primary Care, Oncology, Transplant, Infectious Disease, Neurology (including Multiple Sclerosis), Rheumatology, Dermatology, Gastroenterology, Hepatology, Cardiology, Pulmonology, Allergy and/or various inflammatory conditions preferred.
