We are seeking a compassionate and dedicated Respite Care Provider to join our team, offering high-quality support and care for children with special needs.

p>Position Summary: The After School Provider is responsible for the daily operation and management of the After School Program.

Working as an independent healthcare provider, you''ll play an integral role in our care teams by providing high-quality, cost-effective care to patients while working collaboratively within a multidisciplinary health team. Provide psychotherapy, utilizing multiple modalities and methods consistent with evidence-based practice, including specific and focused treatment protocols, when appropriate.

p>The Intensive Outpatient Services (IOS) Psychotherapist works as member of the IOS multidisciplinary team and is a key provider of direct mental health care for patients and their families/caregivers. Direct treatment functions are designed to meet and exceed the requirements set forth in the Wisconsin Department of Health Services (DHS) Chapter 40 requirements that pertain to a day treatment setting for children and adolescents.

This role collaborates with psychiatrists, healthcare providers, patients, and community resources to develop individualized treatment plans, support recovery goals, maintain accurate documentation, and ensure high-quality, patient-centered behavioral health care. As an integrated health system, we deliver exceptional, coordinated across seven hospitals, 85 primary and specialty clinics, and a team of over 7,500 professionals serving northern Illinois and southern Wisconsin.

Under the direction and supervision of the manager and supervisors from Comprehensive Community Services (CCS) work involves providing comprehensive, person-centered, team based, treatment to children and families living with mental illness, substance abuse or co-occurring disorders, along with services to their families/caregivers. Bereavement Leave (Policy 5.04): In the event of a death an employee may be excused from work without loss of pay for up to a maximum of thirty-two (32) hours annually for the purpose of attending a person's wake, visitation, memorial service, funeral, or make necessary arrangements regarding the person's death, within a reasonable time after the occurrence.

We are seeking a Nurse Practitioner or Physician Assistant to: Be a member of team comprised of a dedicated and talented group of individuals who are committed to patient- and family-centered care, injury prevention work, and keeping kids safe. This includes facilitation of transition from inpatient to outpatient status and facilitation of ongoing outpatient services with third-party providers; provides appropriate outpatient nursing care, and acts as the conduit between primary care and outpatient/community based health care and mental health providers while the patient is a participant in the IOS program.

Position Summary: The Registered Nurse - Maternal Child Health (RN-MCH) works collaboratively as an active member of the interprofessional team to provide therapeutic patient and family centered care for prenatal, postpartum, and newborn patients up to 1 year after delivery. Access Community Health Centers will provide reasonable accommodations to individuals who cannot be vaccinated because of a disability or sincerely held religious beliefs when doing so does not compromise the safety of others or impose an undue hardship on our organization.

We provide psychiatric and chemical dependency services to our patients in a variety of settings, including inpatient psychiatric hospitals, specialty treatment facilities, residential treatment centers, outpatient clinics and therapeutic school-based programs. The hospital offers a range of services, including Partial Hospitalization Programs (PHP), Intensive Outpatient Programs (IOP), Electroconvulsive Therapy (ECT), and will develop specialized programs to address community needs.

p>The Mental Health Therapist OR Clinical Psychologist functions as an independent healthcare provider who, working collaboratively within a multidisciplinary team, provides diagnostic assessment and treatment to applicable demographic population to address the needs of these patients and provide clinical mental health services. Resources will vary based on specific population served and could include, but is not limited to: schools, families, primary care, senior living facilities, and other specialty providers as needed.

Primarily responsible for staff oversight and supervision of the services being provided for children and families enrolled in these Programs, this supervisor assists in development, implementation, and oversight of policies and procedures as well as has a key role in collaboration with internal and external system partners to ensure a trauma informed, family-centered, and strength based approach.

Gathers clinical assessment information via direct client interview and relevant collateral contacts in order to accurately complete WI DHS Mental Health/Alcohol and Other Drug Abuse (MH/AODA) Functional Screen to determine functional eligibility for CRS and need for psychosocial rehabilitation.
