Work in partnership with the treatment team, with participation from natural supports, in the development of the resiliency plan (the intake assessment, functional behavior assessment (FBA)) and evaluation(s) which includes the identification of health care, behavioral health treatment, social services, advocacy, educational, recreational, and community resources to prevent further escalation/decompensation of the individual; link and coordinate services for the individual based on the resiliency plan objectives. Provide updates, regarding client goals and the client’s overall well-being, to the interagency team during meetings and service planning, sharing the resources that have been found for the families, identifying any additional family needs to ensure continuity of care and overall natural support system stability.
