p>The Care Management for High-Risk Pregnancy Case Manager will perform intermediate-level care coordination independently, providing education, referrals, and support to pregnant women with identified risk factors. Education/Experience:

Administer, assist with, and observe medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass ("Right" resident, medication, dosage, time, route, right to refuse). Maintain and clean the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications.
