Job Details
- Position Description: This role assesses patient physical, psychological, social, spiritual, educational, developmental, and discharge planning needs. Responsibilities include reviewing patient history and physicals, diagnostics, and laboratory data, and completing assessments and reassessments.
- Formulates a plan of care: Developing and revising care plans based on assessment data, patient, family, and team input, with realistic and measurable goals in collaboration with the multidisciplinary team.
- Implements the plan of care: Providing direct patient care, coordinating, delegating, and supervising activities of licensed nurses and other personnel while ensuring compliance with policies, procedures, standards of care, and regulatory requirements.
- Evaluates patient responses: Monitoring patient and family responses to care and education, revising care plans as needed, and documenting all phases of care accurately and in compliance with policy.
- Research and education participation: Supporting evidence-based practice, participating in research activities, and engaging in role development and education of facility personnel, patients, families, and visitors.
- Leadership and collaboration: Providing guidance to staff, working with the healthcare team, and interacting effectively with external contacts to ensure quality patient outcomes.
- Physical Demands: Ability to stand, walk, bend, squat, reach, stretch frequently, lift up to 75 pounds, and respond promptly to patient needs, with effective communication skills.
- Qualifications: Must possess a current, valid RN license in the state of practice or an approved compact license, and BLS certification. Additional certifications or continuing education may be required based on practice area. Experience in an acute care hospital setting within the past 12 months is preferred.
