Description

American Traveler is seeking an experienced RN for a Hospice position requiring OASIS and HCHB proficiency for home-based geriatric end-of-life care.

Details

  • Home Health/Hospice setting providing geriatric end-of-life care
  • Day shift schedule, 5x8 hours, 8:00 AM-5:00 PM, working either Tuesday-Saturday or Sunday-Thursday
  • Average caseload of 5-7 patients per day, to be confirmed during interview
  • 40-hour weekly guarantee based on completing 30 visits
  • EMR: HCHB (Homecare Homebase) experience required
  • On-call rotation may be required, to be discussed during interview
  • Territory coverage up to a 60-mile radius from the hub/office, with flexibility expected

Requirements

  • Active WA or compact RN license required, in-hand at time of consideration
  • BLS certification required
  • Minimum 1 year of recent relevant experience required
  • OASIS experience required for staff completing Start of Care (SOC)
  • Proficiency in HCHB charting system required
  • Active driver's license and personal transportation required
  • Skilled in end-of-life care, pain management, symptom assessment, and postmortem care
  • Experience with advanced directives, ethical/legal issues, and patient/family education and support

Additional Information

  • Provide comprehensive hospice care including pain management, symptom assessment, and psychosocial support for patients and families
  • All charting must be completed by 9:00 AM the following morning
  • Timely documentation and communication required throughout assignment
  • Scrubs required in any color; no jeans permitted
  • iPad provided; personal cell phone use not required for the role
  • Mileage reimbursed at the IRS standard rate
  • Monday orientation required, including completion of billable training modules
  • Travel pairs accepted, though each traveler is assigned an individual territory and must have their own transportation
  • First-time travelers will not be considered for this role
  • Local candidates accepted at the travel rate
  • Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
  • Requested time off (RTO) must be submitted at the time of application
