Position Overview
We are seeking an experienced LMSW to provide psychosocial assessment, counseling, resource coordination, and discharge planning for patients and families in an inpatient Med/Surg setting.
Required Qualifications
Master’s Degree in Social Work (MSW) from an accredited program.
Current New York State Licensed Master Social Worker (LMSW) license.
Previous social work experience in a healthcare or hospital setting.
Strong interviewing, psychosocial assessment, and documentation skills.
Strong understanding of discharge planning and care coordination.
Excellent communication and interpersonal skills.
Ability to use sound clinical judgment and make timely decisions in a hospital environment.
Ability to collaborate effectively with patients, families, physicians, nurses, case managers, and interdisciplinary teams.
Knowledge of community resources and social service systems.
Strong organizational skills and ability to manage multiple patients, referrals, and competing priorities.
Commitment to patient confidentiality, ethical social work practice, and patient-centered care.
Responsibilities
Maintain a visible presence on assigned inpatient units and provide timely social work services.
Screen patients within 24 hours of admission to determine the need for social work intervention.
Receive and evaluate referrals from interdisciplinary staff, community agencies, patients, and families.
Complete comprehensive psychosocial assessments within 72 hours of admission.
Develop individualized treatment plans with patients and families.
Provide psychosocial counseling, emotional support, crisis intervention, and appropriate social work services.
Identify and address barriers affecting patient care, treatment, recovery, and discharge.
Develop safe, timely, and effective discharge plans.
Coordinate discharge planning with physicians, nurses, case managers, rehabilitation services, community organizations, and other healthcare professionals.
Connect patients and families with community resources, financial assistance, housing resources, behavioral health services, and other appropriate programs.
Follow up with patients throughout hospitalization to identify changing psychosocial needs.
Advocate for patients and families while maintaining professional ethics, confidentiality, and patient-centered care.
Maintain accurate and timely documentation of assessments, interventions, referrals, and discharge planning activities.
Participate in interdisciplinary rounds, discharge planning meetings, staff meetings, and hospital-wide meetings.
Collaborate with community agencies to support continuity of care following discharge.
Maintain required monthly statistics and departmental reports.
Participate in supervision and professional development activities.
Follow hospital policies, social work ethics, and professional standards.
Perform other related duties as assigned.
