Position Overview
We are seeking an experienced Licensed Independent Clinical Social Worker (LICSW) to provide social work services and psychosocial support for pre- and post-liver transplant patients in an outpatient setting with occasional inpatient support.
Required Qualifications
Active Washington State LICSW (Licensed Independent Clinical Social Worker) license or qualifying licensure as applicable.
Master's degree in Social Work (MSW) from an accredited program.
Clinical social work experience in a healthcare or hospital setting.
Experience conducting psychosocial assessments and providing clinical social work interventions.
Strong assessment, counseling, communication, and documentation skills.
Ability to independently manage a clinical caseload and prioritize competing patient needs.
Experience working effectively within a multidisciplinary healthcare team.
Strong understanding of patient confidentiality, professional ethics, and healthcare regulations.
Ability to provide compassionate and culturally responsive support to patients and families.
Preferred Qualifications
Experience working with organ transplant or liver transplant patients.
Experience in transplant psychosocial evaluations.
Previous experience in an outpatient transplant program.
Experience working with complex medical patients and their families.
Knowledge of caregiver assessment and verification processes.
Hospital inpatient social work experience.
Responsibilities
Complete comprehensive psychosocial evaluations for pre- and post-liver transplant patients.
Assess patients' emotional, social, financial, family, and environmental needs related to transplantation.
Evaluate patient support systems and verify caregiver availability and suitability, including caregiver verification by phone.
Provide psychosocial support, counseling, education, and appropriate resources to transplant patients and their families.
Identify barriers that may affect transplant readiness, recovery, treatment adherence, or long-term outcomes.
Collaborate with physicians, nurses, transplant coordinators, and other multidisciplinary healthcare professionals.
Provide social work evaluations for inpatient patients when required.
Maintain accurate and timely clinical documentation in the medical record.
Assist patients and families with community resources, financial resources, transportation, housing, and other support services.
Advocate for patients and coordinate services throughout the transplant process.
Participate in multidisciplinary discussions and contribute to individualized patient care planning.
Manage a variable caseload, including approximately 10 new patients per week, depending on department volume.
Maintain patient confidentiality and comply with applicable healthcare regulations, organizational policies, and professional standards.
