Responsibilities

Coordinates all aspects of the Stroke Program under the direction of the RN Program supervisor to achieve and maintain the Joint Commission Certification for Community Medical Centers (CMC). Coordinates stroke patient care across the continuum in collaboration with unit leadership, providing stroke-related training and competencies for staff. Coordinates community outreach. Supports monthly interdisciplinary meetings, quality assurance and performance improvement activities for the stroke program.

Qualifications

Education • High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) • Bachelors Degree in Nursing or Healthcare related field (preferred)

Experience • 2 years of relevant experience in the healthcare industry or a healthcare setting (required)

Licenses and Certifications • RN - Current and valid Registered Nurse license to work within the state of California (required) • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) (required) • One of the following is required within 1 year of hire or transfer into position: • SCRN - Stroke Certified Nurse • CNRN - Certified Neuroscience Nurse • ASC - Advanced Stroke Coordinator
