Job Summary
 
Senior Spirit of Roselle Park, LLC Adult Medical Day Center - is seeking Full Time, Bi-Lingual Spanish/English speaking, highly motivated Registered Nurse to join our team.  In this role, you will assist with creating and managing a plan of care for individual patients. You will be required as a Registered Nurse (RN) to operate with great attention to detail and a big heart.  

Primary responsibilities will be supporting the DON with  initial  intake evaluation of New/Transfer patients as well as assisting with discharged members.  Assist with managing prior authorization and maintaining member charts and updating vital record spreadsheets.   

Taking pride in creating personalized care plans for patients and you will be required to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong ability for solving problems. Previous experience working in elder care and with disabled individuals would be highly beneficial.
 
 Under the direction of the Administrator, and the Director of Nursing you shall be responsible for the implementation and coordination of the medical care given at the Center, in addition to assist with managing our LPN & CNA staff as needed.  The qualified candidate will also monitor and report changes in patient symptoms or behavior. Communicate with collaborating physicians or specialists regarding patient care, and facilitate referrals to other healthcare professionals and medical facilities.
 
 
Responsibilities 
  • Coordinate and assist with other colleagues to assess, plan, implement, or evaluate patient care plans 
  • Administer medications and monitor patients for side effects or reactions  
  • Document any changes in patient conditions and report on changes 
  • Maintain accurate records/charts of patient care and discuss observations with supervisor 
 Qualifications
  • Currently licensed Registered Nurse (RN) - New Jersey
  • Excellent interpersonal and client care skills
  • Valid driver’s license 
  • First aid/CPR certification required
