What You'll Do:
Provide direct and indirect nursing care to patients of all ages
Perform nursing assessments and triage activities under supervision
Assist providers and Medical Assistants with patient care, exams, and procedures
Prepare patients for exams and perform and document appropriate screenings
Administer medications, immunizations, and treatments according to provider orders and standing delegation orders
Accurately document nursing assessments, patient education, follow-up, and treatment plans in the electronic health record (EHR)
Perform approved point-of-care testing (POCT) and document results
Review immunization records and reconcile vaccination history
Perform Nurse Only visits and wound care
Triage and process patient calls, messages, tasks, and other communications
Assist with medication refills, prior authorizations, and patient forms
Monitor medication, vaccine, and clinical supply inventories
Follow infection control procedures and properly dispose of biohazard waste
Respond to emergency codes and high-priority lab results
Educate and support patients and families based on their individual needs
Support clinic operations and perform other duties as assigned

What We're Looking For:
Graduation from an accredited nursing program
Current license to practice as an LVN in the State of Texas
New grads encouraged to apply! 0–2 years of clinical experience
Current CPR certification
Bilingual in English and Spanish preferred
Knowledge of ambulatory nursing practices and procedures, adult and child learning principles, and community resources
Strong written and verbal communication and customer service skills
Strong computer skills, including MS Office
Ability to work evenings and weekends as needed
Valid Texas driver's license (or ability to readily obtain) and reliable transportation
Must comply with GCHD immunization policy and ICS training requirements
Must pass a criminal background check and drug/alcohol screening
Equivalent education and experience may be considered.
