Act as the Licensed Practitioner of the Healing Arts (LPHA) for the program, providing clinical oversight and ensuring quality care through chart reviews, determination of medical necessity, level-of-care assessments utilizing approved tools (ASAM), review of problem lists and treatment plans, documentation compliance monitoring, completion of treatment authorizations, intake assessments, admissions, and submission of appeals as clinically appropriate to support continued treatment and service authorization. Facilitate continuity of care through warm handoffs, coordination of transitions between levels of care, discharge planning, and linkage to aftercare services, recovery supports, housing resources, medical and behavioral health services, and other community-based resources that promote sustained recovery and long-term stability.
