Service coordination's purpose is for a collaborative process that assesses, plans, implements coordinates, monitors, and evaluates options and services to meet an individual's health needs through communication and available resources to promote quality, cost-effective outcomes. We are seeking candidates that have a strong background in case management and care coordination, experience conducting in-home assessments and visits, and a demonstrated ability to work effectively with adult populations who have complex medical, behavioral, and social needs.
