Interventions may include, but are not limited to, family support, crisis management, consultations with a staff physicians, Adult or Child Protective Services reporting/coordination, custody/guardianship issue coordination, indigent patient program coordination, substance-use support coordination, intimate partner violence support, bereavement. The Social Worker collaborates directly with the care team, care management team and patient, and/or family, and/or the patient’s representative to assess the patient’s continuum of care needs and ensure the patients transition to the least restrictive care environment.
