REFUGEE CASE MANAGER

Reporting to the CDD Medical Director, this position serves as a key leader and nursing co-manager for the UNM CDD's Medically Fragile RN Case Manager program, specifically leading the Nurse Case Managers providing services in rural satellite offices statewide, providing clinical, operational and staff oversight for all aspects of this large, statewide Nursing Case Management program serving children and adults with medical fragility and developmental disabilities and their families to support their ability to access needed services and support in order to remain safely in their homes and home communities. This role leads staff onboarding, and provides supervision, training and mentorship for the satellite/rural Nurse Case Managers; oversees case assignments and coverage; supports fiscal and operational program aspects via monthly billing review and ensuring accurate and compliant documentation; attends budget meetings and works with the UNM CDD fiscal team to ensure program sustainability.
