- Actively participates in survey/survey readiness activities and performance improvement plans, works to reduce unnecessary patient hospitalizations, improve patient safety, and implements processes and best practices to ensure positive patient outcomes.
Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high quality care to home health patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations. Scheduling requirements: 2 weekends/month, 1 summer holiday (Memorial Day, July 4th, Labor Day), 2 Winter Holidays (Thanksgiving, Christmas Eve, Christmas Day, New Years Eve, New Years Day).
