Clinical Support: Document patient interactions and medication therapy outcomes, including but not limited to: • Accurately gathering medication histories from multiple Electronic Health Records (EHRs) • Identifying potential medication errors or necessary clarification • Reviewing medication use, administration, and prescribing accuracy • Prescribing Authorization (PA) & Medication Refills: Assist with prior authorization (PA) requests and medication refill processing to ensure timely access to medications.
