Responsibilities: As a Hospice Registered Nurse Team Director, you will: ā¢ Supervise direct patient and family care team members, in collaboration with program site leadership ā¢ Responsible for scheduling, payroll, and other administrative duties ā¢ Ensure all initial assessments and required reassessments are conducted by qualified team members within required time frames, including documentation submissions, and/or synchronization ā¢ Oversee and support the Interdisciplinary Group (IDG) in development, implementation, and review of the interdisciplinary plan of care for each patient and family ā¢ Collaborate with program site leadership members to monitor, manage, and mentor staff in patient care activities, productivity, and documentation per related policies and protocols ā¢ Participates and promotes Back to Bedside efforts in reinforcing Case Management skills, and knowledge and attitude per protocol for all disciplines ā¢ Collaborate with site leadership to support staff education efforts ā¢ Facilitates IDG meetings and provides ongoing IDG Support such as triaging incoming calls, provides caller support and assistance, and performs quality calls ā¢ Participates in or lead quality improvement activities, including data collection and aggregation as able ā¢ Reinforces and supports all internal and external communications. If you meet these qualifications, we would love to meet you: ā¢ Associate degree preferred ā¢ Certified Hospice Nurse (CHPN) preferred ā¢ Three+ years experience in hospice, home health, general medical surgery, oncology, or patient population-specific nursing (i.e., geriatrics, pediatric, etc.) ā¢ Minimum of one year of supervisory experience ā¢ Possess and maintain valid CPR certification while employed in a clinical role (preferred) ā¢ Must be a licensed driver who can travel to all business locations ā¢ Meet the regulations and requirements of the state(s) in which the program provides services.
